Pre-register as a New Patient
We are accepting new patients and we take PPO insurance, only.

We need the following from you to Pre-Register you, then we'll send a new patient message where you can finish everything online. We need five things to get started:
1. Full name
2. Address
3. DOB
4. Email address
5. Medical insurance card front & back scan or photo

Please note that we have a 24 hour cancellation fee of $80.00 for missed appts/apts cancelled within 24 hour period. Thank You!

Once you have pre-registered with our practice
1. Set up your MyChart/myCare account by clicking on your activation email
2. Fill out your Patient Medical History on MyChart/myCare prior to your upcoming visit. ✓ This confirms your apt ✓
3. Download and fill out our Patient Registration form (.pdf): New Patient Registration
4. Send (New message/attach) New Patient Registration & ins card copy (both sides) to your MyChart/myCare account
